Is there someone who has gone above and beyond the call of duty to provide a sporting opportunity to others?

How about a team who have beaten all before them or even just done better than expected – perhaps simply winning a first ever game.

Or perhaps a talented young sportsman or woman who deserve credit for an excellent year in their particular sport.

If so, now is the time to make your nomination for the Grass Roots Sports awards.

The Lancashire Telegraph and United Utilities have teamed up to find East Lancs sporting stars.

Nominations have been flooding in but there is still plenty of opportunities for you to have your say - your chance to say ‘thank you’ or ‘well done’ So to nominate your hero , turn to the centre pages to see brief descriptions of all the categories and a nomination form.
