AN internal inquiry into the death of a young man, who is believed to have choked on food at Calderstones, will report by the end of October.
Adam Sykes, 22, originally from the Halifax area, died on September 2 at Burnley General Hospital, despite the efforts of hospital staff and paramedics called to the Whalley hospital.
Calderstones chief executive Russ Pearce said the inquiry was compulsory of health organisations when a patient died in these circumstances.
The review board, headed by non-executive director of Calderstones NHS Trust Neil Beecham, will include independent members and will investigate the following:
circumstances surrounding the incident which led to his death;
all clinical protocols, guidance and treatment in relation to Adam's care at the hospital;
what lessons can be learned;
action taken by staff in relation to the incident.
The report will bodies including Calderdale Primary Care Trust, the area where Adam was from, and the region's top health body the Cumbria and Lancashire Strategic Health Authority.
An inquest into his death has been opened and adjourned.
A post mortem examination was also carried out by a pathologist and the coroner is awaiting the results of tests before a full inquest can be held.
Adam was admitted to Calderstones Hospital in June 2001 from his home in Yorkshire.
