If you haven't already got an application form from us here's your last chance to enter one of the biggest five-a-side tournaments to take place in the region.

The BYAP in partnership with the Audley Community Centre Development team and Asian Image are organising the tournament at the newly opened JJB Sports SoccerDome in Blackburn.

The JJB SoccerDome, Sports centre and gymnasium features state-of-the art fitness and sports equipment.

All forms must reach us by March 15 2005.

Entry Fee: Seniors £40 and £25 for other age ranges

Please make your cheques and postal orders payable to Blackburn Youth Advancement Project.

Each team will comprise of 5 players and two substitutes and games begin at 9.30am prompt.

A full fixture list will be provided on the day of the tournament and players will be able to follow their teams progress on the in-house TV system.

All juniors teams must be accompanied by a manager or parent at all times.

All proceeds will go towards the Tsunami Emergency Appeal.

Normal five-a-side rules and regulations apply.
